I never did a cover letter either. U simply start wd HSL and give a heading as origin of letter where u can briefly describe how u 2 met and the need of this HSL.

We had a cover letter and it included: hubby asking to grant me a pardon and visa back to the US, how we got into this mess, explanation of the attached supporting documents ( like: exhibit A letter from the primary physician, exhibit B financial documents etc.). We also hired a lawyer and she added a legal things ( like compare to XXX case my client should be pardons since her violations are not as big as person in the case).

Waiver itself is just a filled form. I would definitely suggest you to add a cover letter. This is a chance for you to make a good impression to the adjudicator.

    Good luck.

our lawyer wrote a cover letter basically summarizing the main points of our HSL. she included same legal points, such as what immigration laws applied to our case. we also made a separate appendix that listed all the attachments to the 601. in my HSL i started from the beginning, how we met, etc and then went right into the hardships. good luck.
