Change of address form

I'm new to the site and I'm trying to go through all this process and some of it is very confusing. But my question is, on my "Choice of Address & Agent" application, it has my wife's name in Bold letters, and right below it shows a line saying "PLEASE PRINT OR TYPE YOUR NAME", so do I print or type her name or my name? I'm the Petitioner, and she is the one that is applying for a visa, and i'm not.
Then at the bottom it says "SIGNATURE OF APPLICANT", do I sign my name or should my wife sign her name...?

Let me know if anyone has filled this form out thanx.

The choice of agent (DS-3032) is for the foreign spouse (visa applicant) to complete.

If the visa applicant (the one named in bold at the top) wishes to choose someone else as the agent e.g. the US spouse, the first box is checked and the US spouse's full name and address details are completed.

If the visa applicant (the one named in bold at the top) will be the choice of agent, the second box is checked and the applicant completes their address details.

The visa applicant (the one named in bold at the top), signs the DS-3032 Choice of Agent form.
