OK so I was reading through my medical records.....They are not very readable at all. Instead of using my medical records as proof of my prescriptions can I take the labels off the bottles? I have all my old medication bottles incase I ever needed to know the name of the medication so in 2004 when I had Celexa for my depression I could tear off the label and put it in as evidence?

I would contact the pharmacy and simply ask for a printout of all of your prescriptions. You need official, formal evidence.

Ok....You would think that would be official, but maybe not. Geez I have gone to tons of different Pharmacies that is going to be a lot more work, but worth it.

Did you go to the same doctor everytime, I got my medical records for the last 2 years from his office. And the pharmacy did give me a print out of my medications just get a few of the consistant refills as evidence.

Consistant huh? I am not consistant at all. I only get my meds when I can pay for it. I have insurance but even $25 is a lot I think at times, but I will try. I have gone to 7 different pharmacies in the past 7 years so I will just request it from all of them.

I would include your medical records in addition to whatever evidence you have of your prescriptions. More is better than less.

Yes, definitely. If you don't include them, it raises questions as to why they weren't presented. Even if they can't be read, it's clear that you have been seen regularly & repeatedly for a long-time period. Explaining what the records mean is up to you in your HSL, but definitely include the actual records (or photocopies, rather.)

OK I will I saw that picture of your letter. WOW what a big packet. I am going to copy yours if you don't mind I like the way you have it tabbed and layout in that folder like that. I am going to buy my two hole punch and everything today so we will see.

Haha! My waiver was assembled with an ACCO clip. For a while, there was a big to-do about using the exact format that CDJ prefers because it was rumored that they took apart our packets to add things to our files (the files they keep) or whatever. With the Pilot Program, since they only have your waiver for a couple of hours, I think it's less important.
Our letter was 22 pages, the packet was 400 page with 90 items of evidence, plus an appendix, contact list and some other stuff. But since you just sent out your I-130, you've got LOTS of time- I'm sure your's will be stellar.

Well we are now planning on sending out the i-129F and I really think it will be going out this Friday so we may be there in March or April sometime.

in regards to getting proof from medical records....
you can also contact your insurance company..... your insurance company has a list of every medication that you purchased with insurance... it also has a list of all the diagnosis that your doctor billed your insurance for.... of course if you don't have insurance stick with the pharmacy... otherwise try to contact your insurance .. they'll have your medical proof as well
