Affidavits from family

Does anyone have an example for me to look at for the affidavits of a bona fide marriage from a family member or friend? I am trying to write one up for my mother and my friend wants to write one, but I don't know how they should go. Is there a certain form, or just write one up. Must it state "this is a sworn affidavit by XX"? I don't get it. I am sure it is best to have a notary sign it so I got that, but I am just stressed over this because my husband and I don't have a joint lease and don't have joint property, no children and no joint checking. We went to join out checking last night, but he worked uintil 7pm.

i have a suggestion, too, in addition to the joint checking (which you can definitely do at some banks if not all--don't know where you are but we have bank of america, and if you're in the maryland/dc area by any small chance, chevy chase bank works too). you should call all your credit card companies and ask what you need to do to add an authorized user to your account, then save the notification that states that he is one and maybe photocopy his and your cards with the same number or something. i have two credit cards (both visa). my capital one card let me add an authorized user without a ssn--in fact, even before i broached that subject, when they were reading me the terms and conditions, they said, "and the user doesn't even have to be a us citizen or resident..." my wachovia card said no, but one out of two is something. a friend of mine did the same thing with her american express, so i guess it depends on the company. worth a try, though.

Thanks!! We have been trying to add him to my checking, but our work schedules are crazy and the bank is always closed. I will call my CC companies today and see. I have First Premier and I believe they will let me add him on. I just got a cell phone in my name and added him as an authorized user too. Thanks. Anyone more suggestions will be great I am mailing this out next Friday and the Affidavit is written and ready to be signed by my mother and friend. I hope they are right.
